Healthcare practitioners who have registered their information in the online Registration and Licensing System at Department of Healthcare Practitioners may view or print their e-provisional licenses.
Online Instructions
- Select the “Registration and Licensing System” from the e-services list.
- Login with Applicant account username and password.
- Click on “Provisional License”.
- View or print e-Provisional License.
