Description
The Births and Deaths Record Department at the Ministry of Public Health (MoPH) undertakes registering Qatari deaths outside Qatar and issuing of their death certificates, issuing burial authorization for non-Qatari population, sending dead bodies outside the country in addition to other matters.
Offline Instructions
- Download and fill out the paper form, then submit it to the Births and Deaths Record Department.
- Attach all the required documents as follows:
Documents Required For Qatari Deceased in Qatar
- Original ID of the Deceased.
- Original ID of the Informant.
- Notification of Death issued by the hospital.
Documents Required For Qatari Deceased Abroad
- Original ID of the Deceased.
- Original Death Certificate issued at the country where the death occurred.
- Original ID of the Informant.
Documents Required For Expatriates (Non-Qatari Residents)
- Original Passport of the Deceased.
- Original ID / Passport of the Informant.
- Notification of Death issued by the hospital.
