Service Description

This service allows citizen and resident to submit an online request through "Nar’aakom" App of primary health care corporation (PHCC) to change the family physician.

Online Instructions

  • Open Nar’aakom App.
  • Log in to the National Authentication System (NAS) by using your username and password.
  • Choose "Change my Family Physician" service.
  • Enter the applicant name.
  • Select your chosen doctor.
  • Enter the reason for change.
  • Submit your request.

Additional Information

  • You can also apply to this service through PHCC website.
  • Select single physician from the list of Family Physicians shown for your Current Assigned Health Center.
  • If the applicant already has an application in process for this eservice, you will not be allowed to submit another request for the same applicant unless the pending request is cancelled/resolved.
  • Family Physician assignment will be confirmed within 48 hours based on physician availability, and there is no guarantee that the selected physician will be assigned unless approved by the e-services team.
