Request Destruction of Imported Foodstuff
Food importers and clearance companies may submit a request to the Food Safety Department at the Ministry of Public Health (MOPH) for reanalysis of foodstuff.
Offline Instructions
- Download and fill out the paper form, then print it out and submit it to the Ports Health and Food Control Section.
- Attach the following documents:
- Rejection certificate
- Letter from importer justifying the request
- Copy of customs declaration
- Copy of foodstuff reservation form
