Service Description

This service enables health practitioners to electronically submit a sick leave request for their patients, or employees can apply online on behalf of the health practitioner.

Online Instructions

Applying by Health Practitioners:

  • Click on the Issue Electronic Sick Leave (e-Jaza) link. 
  • Select Registration & Licensing System from the e-services list.
  • Sign in with your " Practitioner " email ID.
  • Click on “Create and Submit Requests” and then select “eJaza”.
  • Fill in the “Patient Details” in all the required fields, and “Leave Details”.
  • Submit the request and then download it to the health practitioner's computer. 
  • Click on "issued e-Jazas ” from the drop down list to view already issued electronic sick leaves.
Appling by Employer on behalf of health practitioner:
  • Click on the Issue Electronic Sick Leave (e-Jaza) link. 
  • Select Registration & Licensing System from the e-services list.
  • Sign in with your "Emplyer" email ID and password.
  • Click on “Create and Submit Requests” and then select “eJaza on behalf of health pratitoner”.
  • Select the practitioner then click on “Issue eJaza”
  • Fill in the “Patient Details” in all the required fileds, then the “Leave Details”.
  • Submit the request.
  • To print the issued e-Jaza from the health practitioner, select e-Jaze from the drop down list, then enter the patient name and print the report. 

Additional Information

  • This Report cannot be used as an e-Jaza certificate.
  • After printing the eJaza, sign it and stamp it with your practitioner and facility stamps.
