Service Description

Private sector companies may submit a request to the Maritime Transport Planning and Licensing Department at the Ministry of Transport (MOT) to issue and renew this maritime document, which is given to seafarers working on big vessels. This document is given only to Qatari seafarers.

Offline Instructions

• Select "Seaman's Discharge Book" service from the "Big Vessels" list, available under Maritime Transport services on MOT website.
• Click on the "Download" button.
• Fill out the application form, and submit it to the Maritime Transport Planning and Licensing Department.
• Attach the following documents:

• • A formal letter from the company addressed to Department Director
  • 3 copies of Marine Certificate
  • Seafarer Medical Fitness Certificate
  • Copy of the Passport
  • Copy of QID card
  • Police Clearance Certificate issued by Criminal Evidences and Information Department
  • (2) colored and new photographs (without any headdress)
• Please send the application form with the required documents through the Maritime Transport Planning and Licensing Department's email address: maritimetransport@motc.gov.qa.
