Service Description
Healthcare practitioners or healthcare facilities may submit a request to Department of Health Professions (DHP) to approve deactivating the existing medical licenses of the practitioners and their files in the e-registry once they leave a healthcare facility or the State of Qatar.
Online Instructions
- Log in to the Medical Licensing Registration System.
- Click on “Apply for Removal from the Registry”.
- Save “Personal Declaration”.
- Click on “Apply for Removal from the Registry”.
- Save “Personal Declaration”.
- Fill in all the required information as per the highlighted asterisks.
- Complete the application attaching all the required documents, then submit it.
- After submission, your request will show (Pending with employer) which means your authorized focal point should approve your request via the institution account.
Additional Information
- The employer focal point shall approve the request through the institution account.
- Please check and keep up with the circulars issued by the DHP to get the latest updates of the registration/licensing requirements.
