Description

This service allows you to submit requests to obtain medical information about you or on behalf of a family member or any other person, provided that there is an authorization and permission to do so.

Online Instructions

• Enter the Hamad Medical Corporation website, and select the “Apply for Medical Report” service on the main screen.
• Fill in the health card number, then enter the passcode that is sent once to the phone number registered with Hamad Medical Corporation.
• Enter a valid email to send clinical information as requested.
• Write the name of the hospital, the type of information required, the choice of department or specialty, the time period and the reasons for the report.
• Choose the method of submitting the medical report (Qatar Post – in person attendance).
• Identify the applicant's data (the patient himself, parent/guardian, agent).
• Upload a front and back photo of the patient's ID card through the device that is used to submit the application.
• Upload a copy of an official authorization letter from the patient in the name of the person who will receive the report on behalf of him.
• Pay the application fees (a credit card (Visa) must be available, it is not possible to pay with an ATM card).
• The application number will be sent to the patient to follow up on the status of his application electronically.
