Description

Specialist physicians may apply to perform medical or surgical privileges by submitting a request to the Registration Department at Qatar Council for Healthcare Practitioners to be granted the right to perform such privileges in the places of work mentioned in their medical licenses or in any other healthcare institution inside Qatar after the physician gets the required approvals from this facility.

Offline Instructions

  • Download and fill out all relevant paper forms, then submit two copies of each after signing them to the Ministry of Public Health.
  • Attach the following documents, making sure to prepare two copies:

  • Surgical/ Medical Privileges Form for the specific specialty (marking in the requested column) and signed by both the physician and the medical director of the facility.
  • Two copies of valid medical license or copy of valid evaluation.
  • Request letter (cover letter) signed and stamped by the medical director of the place of work, specifying the start date of work, good standing and the name of the facility where the applicant wants to perform the surgeries.
  • Request Letter from the facility where the applicant wants to perform the surgeries (it could be in a different workplace from the one mentioned in his/her license).
  • Stamped approval letter from the facility where the applicant wants to perform the surgeries (available on the website).
  • Copy of official surgical log book for the last three years (must be attested by the official competent authorities).
  • Personal Declaration for Surgical/ Medical Privileges.
  • Copy of bachelor degree or its equivalent and copy of the specialization certificate or its equivalent.
  • Updated Curriculum Vitae (C.V.)
  • Recommendation letters (if any)
  • In case of applying for new additional privileges, the request should be submitted after a period of six months from the first request along with all the aforementioned requirements in addition to the following:
  • Justification letter for applying for additional Surgical/ Medical Privileges.
  • Copy of new training certificate/courses attended in the requested privilege.
  • Copy of new work experience in the requested privilege (log book).
  • Copy of previous Surgical/ Medical Privileges approval letter from the Registration Department.

Additional Information

  • This service requires 25 working days to be completed, and applications can be submitted on Mondays and Wednesdays from 8:00 am to 12:00 pm.
  • Medical/ Surgical privileges can be granted to specialist physicians only as per the approved policies.
  • Any incomplete request will not be processed.
  • Arrange your application according to the above-mentioned requirements order.
  • Original attested surgical log book must be checked by the perspective medical licensing officer (in the final stage).
  • Documentation necessary to support this application must be maintained and presented upon request.
  • Original documents (certificates) must be submitted upon request.
  • Presented documents must be translated into Arabic or English (attached to copy of the original documents).
  • MoPH provides forms for surgical privileges of different specialties.
