Service Description
Dentists seeking to apply for dental privileges shall submit a request to the Ministry of Public Health (MoPH), completing all the required information and attaching all the necessary certificates.
Offline Instructions
- Download and fill out the paper form, then submit it to MoPH.
- Attach the following documents:
- Request letter from the place of work mentioning the requested privilege.
- Copy of work experience certificate in the request privilege.
- Copy of training certificate/ courses attended in the requested privilege.
- Curriculum Vitae (C.V.)
- Copy of valid medical license.
- Personal Declaration of dental privileges.
- Copy of bachelor’s degree or its equivalent.
- Copy of post graduate degree certificates.
- Any other additional requirements requested.
Additional Information
- Any incomplete request will not be processed.
- Any other required documents that are not mentioned above shall be submitted upon request.
- Original documents/ certification if required shall be submitted upon request.
- Presented documents must be translated into Arabic or English (attached to copy of the original documents).
- Dentists who want to apply for Privileges to Do Dental Treatment Under General Anesthesia shall submit a request to the MoPH.
- You may follow up on your request after a minimum of 20 working days.
